Medical Foster Home Program

 

The Medical Foster Home Program (MFH) is a voluntary program that offers safe, long-term care in a home-like setting for VA Healthcare eligible Veterans. The MFH caregiver resides in the home and provides 24/7 care for the Veteran. MFH is an affordable, private pay, comprehensive care housing alternative for Veterans.

Services Provided at Medical Foster Home

  • Furnished private or semi-private room
  • 24-hour care
  • Medication management
  • Personal care (dressing, grooming, bathing etc.)
  • Meals, laundry, cleaning
  • Flexible daily routine
  • Long-term care option
  • Signed care agreement between caregiver and the Veteran or legal representative
  • Personalized care
  • VA Recreation Therapist

VA Provides

  • Home oversight through annual home inspections & unannounced compliance visits
  • Assistance with maximizing VA benefits, when eligible
  • Case Management through a Home Based Primary Care team: Nurse Practitioner, Nurse, Social Worker, Dietitian, Psychologist, Physical Therapist/Occupational Therapist

Medical Foster Homes & Caregivers

  • All Caregivers must provide proof of background clearance
  • Home must pass annual inspection by the VA Inspection Team
  • Home must be owned or rented by the caregiver
  • Caregiver must be 21 years or older and financially stable
  • Caregivers are flexible and have problem solving skills
  • Caregivers live in the home and are experienced with providing daily hands-on patient care
  • Caregivers are physically able to provide the needed care based on a physician’s medical clearance
  • Caregiver must have written backup plan if unable to provide care
  • Caregiver ensures a welcoming, family-like atmosphere
  • Caregivers must work collaboratively with the VA Home Care team
  • Caregiver completes annual training
  • Caregivers are CPR certified

Funding

  • Private pay- average rates are based on care needs: $2000 to $3500We currently have homes in the following areas:

  • Zanesville
We are currently recruiting for homes and caregivers, particularly in the following areas: The Greater Columbus Area, as well as Madison, Marion, Delaware, and Licking Counties.
